Medicare pays the office visit codes with place of service 02 or 10. Florida Blue's commercial plans moved to 98000 to 98015 on January 1, 2026.
You see medication patients by video, and each visit needs a code: 99214 or 98006. Check each payer's telehealth policy before you set the code.

Office visit codes, for Medicare and Florida Blue's Medicare Advantage plans
Medicare pays your video visit as an office visit. New patients take 99202 to 99205. Established patients take 99212 to 99215. Your claim carries a place of service (POS) code, 02 or 10, from Medicare's telehealth booklet, and our place of service guide shows which one fits.
Medicare doesn't pay 98000 to 98015. Medicare's 2025 fee schedule rule marked the codes not valid for Medicare, and the 2026 rule kept that status.
Medicare allows an audio-only visit, with modifier 93 on the claim, when your patient is at home and can't use video or doesn't consent to video. Florida Blue's Medicare Advantage plans take the office visit codes too, per Florida Blue's virtual visit policy. For those plans, add modifier 95 for a video visit or 93 for an audio-only visit.
Telemedicine codes, for Florida Blue's commercial plans
Florida Blue's commercial plans moved to the telemedicine codes on January 1, 2026. If you're in Florida Blue's network, your video visits take 98000 to 98007. Your audio-only visits take 98008 to 98015. Don't bill an office visit code with modifier 93 or 95 for your Florida Blue commercial patients.
Pick the code by your medical decision making or your total time. New patients take 98000 to 98003. Established patients take 98004 to 98007. You'd bill 98006 for an established patient with moderate medical decision making, the level of a 99214, or with 30 to 39 minutes of total time.
