What your 90833 actually pays: Medicare's 2026 rate for the add-on prescribers forget

A prescriber who does 20 minutes of real psychotherapy inside a medication visit can bill for it separately. Medicare raised the rate 12.0% for 2026, the largest jump of any visit or therapy code in this series, and it is still the code most solo PMHNPs (psychiatric mental health nurse practitioners) forget exists.

The code, and the rule that makes it easy to miss

CPT code 90833 is psychotherapy, 30 minutes, billed as an add-on to an evaluation and management (E/M) visit like 99213 or 99214. It cannot be billed alone. Report it without a primary E/M code on the same claim and it is automatically denied. It exists for exactly one situation: a prescriber does genuine medication management and a separately identifiable, separately timed block of psychotherapy in the same appointment, and documents both.

A solo psychiatric mental health nurse practitioner (PMHNP) in Massachusetts, running behind on notes and using a dictation tool that was not built for behavioral health, described the problem plainly on a discovery call: she wanted to start adding the add-on code for brief therapy she was already doing inside med management visits, but had no way to check whether her note actually documented the extra time well enough to support the charge. She suspected she had been leaving that revenue on the table for a while. She was right to suspect it. The add-on code requires nothing exotic, just a note that clearly separates the medical decision-making time from the psychotherapy time, and most templates built around medication management alone never prompt for that second block.

What Medicare pays in 2026

Medicare pays $81.50 nationally for 90833 in 2026, up from $72.78 in 2025, an increase of 12.0%, the largest gain of any standard office-visit or therapy code in this series (only the two Spravato administration codes later in this series rose faster, on a different mechanism, drug pricing rather than the conversion factor). The number underneath that move is the Medicare conversion factor, a single dollar multiplier that CMS applies to every code on the physician fee schedule. It fell for five straight years, from a peak of $36.09 in 2020 to $32.35 in 2025, before this year's rule reversed course: $33.40 for 2026, a 3.26% increase. Behavioral health and time-based codes were explicitly exempted from a separate 2.5% efficiency cut applied elsewhere in the same rule. Stacked onto a 2026 99214, a 90833 add-on brings the combined visit to $217.11, versus $135.61 for the E/M visit alone.

What commercial payers pay

PayerPrice shows national average commercial rates for 90833 from $72.97 (BCBS/Anthem) to $111.45 (Cigna) as of August 2026, all reasonably close to the 2026 Medicare rate, unlike the wider spreads seen in some other codes in this family.

The so-what

  • Check whether your template even asks. If your note-taking workflow has no field for a separate psychotherapy time block during a medication visit, it is structurally impossible to bill 90833 correctly, no matter how often you actually do the work.
  • Time the two parts separately. The add-on requires the psychotherapy portion to be identifiable on its own, not folded into a general narrative of the visit.
