UnitedHealthcare's plans take claims at different payer IDs
UMR, Surest, All Savers, and Golden Rule each have their own ID. Oxford plans can take 06111 or 87726, and Optum's behavioral health claims go to 87726.
Your patient's card says UnitedHealthcare, UMR, or Oxford, and your claim came back rejected or paid months late. Look up the plan's payer ID before you send your next claim.

- Read the plan name on the card. UMR, Oxford, Surest, All Savers, and Golden Rule are UnitedHealthcare affiliates, and each affiliate can carry its own payer ID on your claim.
- Find the plan's row on UnitedHealthcare's payer list. The claims payer list, dated May 13, 2026, also shows former IDs for some plans, so you can spot an old ID in your billing system. Match the full plan name. One brand can appear on more than one row.
- Send Optum behavioral health claims to 87726. You'll find 87726 on the list's row named UnitedHealthcare too. Optum's Provider Express site gives the same ID. The list shows 33053 as Optum behavioral health's former ID. You don't need a payer ID on paper or when you enter the claim on Provider Express, which is free for clinicians in Optum's network.
- Send a rejected claim again, to the plan's listed ID. Don't reuse your old payer ID. Get the newest edition of the list, and check whether your plan's row carries an asterisk, the list's mark for a change.

Payer IDs change. UnitedHealthcare Community Plan of Ohio, a Medicaid plan, moved to 88337 from 87726. Plan names change too: Surest was called Bind, so a patient's old Bind card takes Surest's ID, 25463.
Optum's employee assistance program claims go to 87726 too, with modifier HJ on the CPT code, per Optum's Provider Express site. Your patient's UnitedHealthcare Shared Services card takes 39026, the same ID as UMR, not 87726.
JotPsych's Scan Card marks a plan Active only when the scan links the payer by its payer ID.
